We are currently accepting new patients. Please feel free to call during office hours to make your first appointment.
 
Your initial visit will consist of a cleaning, any necessary radiographs and a thorough exam. During this time we will discuss your diagnosis and all treatment options. Some complex treatment plans may require a second consultation visit.
 
New patient forms required for your first visit can be downloaded and printed from our website below. Please bring these completed forms to your first visit.  If you would rather fill these forms out at our office please arrive 10 minutes before your scheduled appointment to complete them.  These forms consist of patient information, insurance information and a medical history form.  Please include any current medications and allergies as these can influence your oral health and treatment options.  
 
We know your time is valuable and every effort will be made to see you at your scheduled appointment time. Therefore, it is very important to us that our patients arrive on time as well. We are fortunate to have patients in our practice that will give a minimum of 24 hours notice if they have a change in their schedule and are unable to make their appointment time. We ask you do the same. With this advanced notice, we will be able to contact another patient who may be awaiting treatment.
 
If you are in pain or feel you have a dental emergency, please contact the office as early in the morning as possible and every effort will be made to see you the same day. We look forward to meeting you soon!
